Elder Care Services

Assisting seniors with daily activities to maintain their independence, safety, comfort, and dignity.

Supplemental Support in Community Living

Providing personalized support to individuals who live in Independent or Assisted Living communities, and CCRCs—helping them remain safe, engaged, and independent without needing to move to a higher level of care.

Personal and Companion Care

Offering assistance with personal tasks (such as light housekeeping, meal preparation, bathing, etc) and social and emotional support to improve quality of life.

Brain and Mobility Support

individuals living with physical disabilities (such as Parkinson’s disease or vision loss), cognitive decline (including dementia or Alzheimer’s disease), and acquired brain injuries (such as stroke or traumatic brain injury) by delivering personalized care that supports their safety, independence, and overall well-being in daily life.

Respite Care Services

Giving family caregivers a much-needed break by taking over the attentive care of their loved ones.

Family and Friend Caregiver Services

Guiding families and friends through Maryland’s Medicaid HCBS Waiver programs, mainly Community First Choice (CFC) but also CPAS, CO, and ICS, which help to support and compensate eligible family and friend caregivers for the care they give to a relative, neighbor, or friend.

Steps To Receiving Home Care With Medicaid Coverage

Start Your Application

Call the Maryland Department of Health at 410-767-1739 to start your application.

Select Your Support Planning Agency

Choose a Support Planning agency to help navigate the process (or one will be assigned if no selection is made within 21 days).

Complete Your Needs Assessment

Complete your needs assessment when the Local Health Department calls you.

Develop Your Plan of Service

Work with your support planner to develop your Plan Of Service (POS) by requesting the type and amount of services you need and listing Concierge Home Care of Maryland as your personal assistance provider.

Your POS is Reviewed

Maryland Department of Health reviews your plan of service for approval.

Receive an approval letter with your official start date.

In-Home Assessment

Schedule an in-home nurse assessment and complete necessary paperwork.

Steps To Receiving Home Care With Medicaid/CFC Coverage

Start Your Application

Call the Maryland Department of Health (844-627-5465) or Maryland Access Point (844-627-5465) to initiate a referral for the HCBS Waiver/CFC program.

Select Your Support Planning Agency

Choose a Support Planning Agency (or one will be assigned if no selection is made within 21 days).

Complete Your Needs Assessment

Complete your needs assessment when the Local Health Department calls you. Schedule and complete a functional needs assessment when contacted by the Local Health Department or Telligen (a state contractor). 

Develop Your Plan of Service

When your support planner calls, develop your Plan Of Service (POS) by requesting the type and amount of services you need and listing Concierge Home Care of Maryland as your personal assistance provider.

Your POS is Reviewed

Once your POS is approved, you’ll receive an official start date letter.

Frequently Asked Questions

Eligibility depends on factors such as age, medical conditions, and level of assistance required. Medicaid clients must go through an assessment process, while self-pay clients can begin services immediately.
Yes! You may select a caregiver you know if you qualify under the CFC program, or we can match you with a trained, professional caregiver from our team.
Self-pay clients can begin services quickly after the in-home nurse assessment. Medicaid and CFC clients must go through an approval process, which can take several weeks to months, depending on program requirements.
Yes. There are limitations to the services covered by Medicaid and CFC program clients. For instance, services such as companionship, supervision, home exercise, emotional support, recreation, and technology support are not covered by Medicaid and the CFC program. Self-pay clients however can receive these services when needed.
