How To Refer a Client
Whether your client needs care within 24 hours or you’re exploring home care or CFC service options, we’re here to help. A member of our team will follow up promptly to discuss your client’s needs and answer any questions you may have about your client’s eligibility, our services, care plans, or approach.
What to Expect When Sending a Referral

Use the Referral form below to send your and your client’s basic information. You can call us at 410-921-7030 or fax the information over to 410-995-8092.
Consultation Call
Our team will reach out to you to gather more detailed information, including the client’s care needs, preferred schedules, and the type of payer source (private-pay, Medicaid, or CFC).
Client Eligibility Check (For Medicaid/CFC Referrals)
For private-pay clients, we proceed by discussing their preferences, budget, and scheduling needs. For Medicaid/CFC clients, our team coordinates with the support planner to verify eligibility and receive the required documentation.
In-Home Assessment
Our nurse conducts an in-home safety and needs assessment before the start of all services, both for private-pay clients and Medicaid/CFC clients.
Caregiver Matching (or Onboarding for Family & Friend Caregivers)
Once our nurse creates the care plan, we match the client with a caregiver who meets their needs. For clients bringing their own caregiver, we onboard their family/friend caregiver by handling the necessary training and completing the hiring process.
Start of Services & Ongoing Communication
Once the caregiver is assigned or onboaded, services begin, and we maintain open communication with the healthcare professional, client, and caregiver. We ensure the care plan is followed, make any necessary adjustments, and conduct regular check-ins to uphold high-quality care and address any changes in the client’s needs.
